Qualcomm Life, a subsidiary of wireless technologies giant Qualcomm, announced a collaboration with ResMed, King’s Daughters Medical Center and CareCentrix to develop connected health solutions powered by Qualcomm Life’s 2net and HealthyCircles platforms.

The 2net platform offers a set of wireless health solutions that can capture and deliver medical device data to integrated portals or databases from nearly any customers’ or technology partners’ wireless medical device for storage.

The platform is a cloud-based system designed to be universally interoperable with different medical devices and applications, enabling end-to-end wireless connectivity while allowing medical device users and their physicians or caregivers to access biometric data.

“The home is the fastest growing care setting in the United States–because of that, the demand for wireless health technology is increasing and health care organizations are looking for opportunities to support patient health and monitoring outside of hospitals and traditional care facilities,” Rick Valencia, senior vice president and general manager of Qualcomm Life, told eWEEK.

Valencia said connected health care technology solutions are crucial for transitional care, remote patient monitoring, patient medication adherence and ultimately—and most probably most importantly–engaging patients in managing their own health.

The HealthyCircles platform is a hosted enterprise software-as-a-service (SaaS) solution that provides a secure communications and record-sharing infrastructure for care team coordination, post-discharge transitional care and complex condition management.

The private-label branded platform is designed to optimize accountable models of care with exception-based management tools to expand remote care capacity, deliver targeted interventions and manage at-risk patient populations.

ResMed, which develops medical equipment for sleep-disordered breathing and other respiratory disorders, is using Qualcomm Life’s technology to streamline transitions of care from the hospital and into the home.

The company is integrating the 2net Hub and Platform with its Astral 100 and Astral 150 life support ventilators to capture and transfer near real-time biometric data to the HealthyCircles care coordination platform, which allows care teams to more effectively monitor and manage patients by exception.

ResMed is also using HealthyCircles in combination with its continuous positive airway pressure (CPAP) device to enable hospitals and care providers to more closely monitor patients to prevent hospital re-admissions.

Focused on patients with heart failure, chronic obstructive pulmonary disease (COPD) and asthma, KDMC’s integrated care coordination and management services.

The organization plans to increase touch points in the rural community and link community-based providers through the 2net and HealthyCircles Platforms by collaborating with Qualcomm Life to deploy and scale its health services to manage employers’ high-risk patient populations with complex care needs in Kentucky, Ohio and West Virginia.

“The whole health care IT industry is in transition right now, which is why adoption up to this point has taken a long time. Population health management has replaced individual fee-for-service models as the goal for health care payers, providers and organizations,” Valencia said. “The next big steps are accelerating adoption, establishing evidence and defining successful mobile health roadmaps so we can move connected health technology from novel technology to a standard of care.”
