Tech giant Qualcomm’s health care-oriented subsidiary, Qualcomm Life, announced it is collaborating with P2Link and Davita Healthcare Partners to develop chronic care management solutions powered by Qualcomm Life’s 2net Device Connectivity Platform and HealthyCircles Care Coordination Platform.

Designed to enable continuous care, informed interventions and better management of at-risk populations, the partnerships also include an expansion of an existing health monitoring program in Arizona.

P2Link is a provider of health monitoring solutions, enabling payors and providers to help reduce costs while improving quality of care for patients with heart failure and chronic obstructive pulmonary disease (COPD).

The company is collaborating with Qualcomm Life in the United States and Europe to deploy remote monitoring medical device kits powered by the 2net Hub and Platform for patients suffering from heart failure and COPD, with the first patient enrollment expected to begin in the United States in the fourth quarter of this year. P2Link’s first implementation of 2net will be with Emerson Hospital in Concord, Mass.

Through its pay-for-performance services that leverage proprietary predictive models and an in-house clinical call center, P2Link targets to reduce the cost of care for heart failure and COPD patients by 10 to 15 percent, helping payors and providers as they transition to a value-based reimbursement system.

Davita HealthCare Partners, a manager of medical groups and physician network operators, provides integrated care management for approximately 830,000 patients, more than 600,000 of whom live in California and represent a range of cultures, socioeconomic groups, ages and health statuses.

The company will integrate Qualcomm Life’s 2net Mobile and HealthyCircles technologies into a coordinated care program for patients with heart failure in a program that will also be rolled out in the fourth quarter of the year.

Patients will be equipped with a home blood pressure monitor, a weight scale and a tablet powered by 2net Mobile to seamlessly collect and transmit the biometric data to the cloud.

Patients will also leverage the HealthyCircles Platform to monitor their health status at home, and to receive relevant educational content and targeted, proactive care management interventions.

Meanwhile, Northern Arizona Healthcare (NAH), the parent organization of Flagstaff Medical Center, has expanded its home health monitoring program, Care Beyond Walls and Wires, to include more segments of NAH’s patient population.

The expansion includes the use of individualized remote monitoring kits powered by Qualcomm Life’s 2net platform and HealthyCircles to help enable team-based care coordination and near real-time patient monitoring whether patients are at home or on the go.
