Private investment firm Comvest Partners, with co-investor Mosaic Health Solutions, announced it has completed the acquisition of McKesson Corp.’s Care Management business, which will be renamed AxisPoint Health.

AxisPoint’s offerings include complex and chronic case management services, nurse advisory services, and a care management software platform used by payers to manage utilization, disease, and case management services in-house.

The company provides its services through nurses and other clinical professionals in 24 states.

“The care management business that was acquired from McKesson and has now been renamed AxisPoint Health was rather unique in McKesson, delivering member-level services and provision of clinical care,” Otto Campo, chairman and interim CEO of AxisPoint Health, told eWEEK. “McKesson’s decision to divest the business was driven by strategic portfolio management.”

Campo said Comvest, in partnership with Mosaic Health Solutions, decided to purchase the business based on its sound financials, experienced professionals, and strong track record of performance for its customers.

“Given the change in the healthcare delivery solution environment, which is increasingly focused on performance outcomes, Comvest felt that investing in a business experienced in chronic care management delivery solution systems is critical to providing health plans and risk-bearing entities the capabilities to manage their most complex members,” Campo said. “Comvest has a vision for growing the business, and views acquiring AxisPoint Health as a foundation to expand into other markets and add complementary assets.”

Campo said AxisPoint Health would build on its key strengths, such as improving health outcomes, simplifying complex care, and directing members to the right care at the right time.

“As the healthcare reimbursement environment is changing, cost saving technologies, complemented by improved quality and health outcomes, are crucial for payers and other risk-bearing entities,” Campo said. “Intelligent technologies will determine the appropriate level of care that a member needs given his or her specific diagnoses or conditions. The right technologies serve to support the patient’s journey through the continuum of care, and point them in the right direction to access the right level of care at the right time.”

The company’s Vital platform is a care management workflow tool designed to improve care coordination, communication, and decision-making. AxisPoint also offers a 24/7 nurse advice line, among its other chronic condition management and complex case management solutions.

“Historically, care management solutions have been driven more by addressing episodic events or health issues,” Campo said. “In contrast, chronic condition management is about identifying potential conditions early and treating them with a holistic approach, using data analytics to develop intelligent member care plans.”
