Social health management company Welltok announced the launch of CaféWell Connect, its partner program. The launch unites condition management and wellness programs with select applications and tracking devices into one platform, the CaféWell Health Optimization Platform.

CaféWell Connect partners will have the ability to reach millions through Welltok’s existing client relationships, including four out of five of the nation’s top payers and health systems. Partners also receive sales and marketing support as well as analytics and activity reporting services.

CaféWell Connect will launch with select partners in six key areas, including condition management and wellness programs initially focused on chronic obstructive pulmonary disease (Propeller Health), prenatal (Wildflower Health) and depression (Brain Solutions [BRC: ASX]) and health tracking devices (Fitbit, FitLinxx, Jawbone).

Other areas include biometric and health risk assessment services (Provant, Hooper Holmes, Tonic Health), mobile applications (MapMyFitness, Azumio), organized communities (Crohn’s & Colitis Foundation of America) and health-related content and programs (Greatist, Health Nuts Media).

The end-to-end enterprise solution provides managers with detailed accounting, reporting, member eligibility information and technical support for operational efficiencies, while adhering to high levels of security and privacy.

“Personalized health care has really exploded, which speaks volumes to the demand and opportunity, but it has also created a fragmented marketplace,” Maneesh Goyal, senior vice president of corporate development for Welltok, said in a statement. “We are solving the overload challenge by providing population health managers with the ability to seamlessly integrate best of breed solutions with existing client programs in an efficient and engaging manner.”

Population health managers, including payers, health systems and accountable care organizations, can select from a curated list of partners to plug into the CaféWell platform. Existing enterprise programs and services may also be added to provide an integrated platform as service solution for administrators and users.

“We are aligning with market and technology leaders who have proven the ability to build engaging, effective solutions for health consumers,” Goyal added. “We plan to continue building the CaféWell Connect program in the coming months and creating an unrivaled ecosystem.”

In November, a partnership with IBM was announced to bring a collaborative solution called CaféWell Concierge powered by IBM Watson to market. The solution offers health care consumers the ability to identify personalized activities, health content and condition management programs to optimize their health and get rewarded for positive behavior change.

The is sponsored by health plans, health systems, accountable care organizations, health retailers and other entities that are seeking to improve consumers’ health status and lower costs. Welltok is also working with industry leaders such as Aetna to help shape the product direction and pilot test the product with members.
